Do I need to create a custom user group and a dedicated access level just for that user, or is there a more efficient built-in option in jDownloads for assigning per-user download permissions without affecting the rest of my site's ACL structure?

Access and UsergroupsI am not quite sure what you are asking but the jD usergroup settings are independant of the Joomla ACL settings except for the priority from super user to public user. A useful starting point is the article
